FAQ - Do you have an example of a 'Student Consent' form for audio visual materials using students (e.g. photographs)?

Answer

If you wish to use photographs, video footage or sound recordings of your students in lectures, text books, journal articles or on the world wide web, you must ask the student for their written consent.  MEDEV has created a template student consent form based on an original document from Oxford Medical Illustration, John Radcliffe Hospital, Oxford, UK which you can download in pdf format.
